How can i change my best friends last name to my dads?

well were both 17, we've been attached at the hip since we were 6, and now i'm finally changing my last names to my dad along with my mom and best friend. shes been part of the family forever and we just want to make it known were a family. so my question is what do we do to change her last name to ours? we live in las vegas btw
